Project summary
In our Phase 1 study, we designed an intervention to support and empower families as they and their baby transition from the neonatal intensive care unit (NICU) to their homes. As part of the intervention, we have pioneered the role of the nurse navigator, who helps families navigate the healthcare system and provides psychosocial support. In Phase 2, we are studying how to implement the effective components of the nurse navigator intervention across Canada, in preparation for wide-spread implementation.

Audience: Families with a new baby who are transitioning home from the NICU.

The CHILD-BRIGHT ADAPT team has prepared infographics sharing the findings from CCENT. The infographic has been presented at the Infant and Early Mental Health, Expanding Horizons, conference in May 2025.
